January 2011: Case Scenario
A patient is being admitted for home care services due to 3 falls in the past month, uses a four-wheeled walker (FWW), smells strongly of urine, looks disheveled and poorly-groomed. He lives alone. His laundry room is in the basement of his small home. He has a son that visits him about once a week to bring him groceries. He takes Celebrex, Lisinopril, Lasix, Namenda and Prilosec. He is unable to tell you what the medicines are for and if he remembers to take it regularly. Based on the information provided, what diagnoses will you use to justify home care, what services will you provide and why?
